Unite Pennsylvania is a coordinated care network of health and social service providers. Partners in the network are connected through Unite Us‘ shared technology platform, which enables them to send and receive electronic referrals, address people’s social needs, and improve health across communities. Unite Pennsylvania is sponsored by Philadelphia Youth Network. JEVS Human Services is a network convener. The network is supported by a Pennsylvania-based Unite Us team focused on community engagement, network health and optimization, and customer success.
Joining the network is at no cost for community-based organizations and many organizations that are considered part of the safety net, like community health centers, tribal clinics, and mental health centers.
Unite Arizona serves partners in Maricopa County, and is launching in February 2021.
Unite Pennsylvania serves partners in Allegheny, Blair, Bucks, Butler, Cambria, Centre, Chester, Cumberland, Dauphin, Delaware, Erie, Lehigh, Luzerne, Montgomery, Philadelphia, Washington, and Westmoreland counties.
JEVS Human Services has partnered with Unite Us to join the Unite Pennsylvania network as a convener. Together, we launched “Re-Unite Us,” a program focused on enhancing opportunities for the reentry community, pre-trial detainees, and sentenced inmates by connecting them to key resources in the Unite Pennsylvania network.
